| Change of Dismissal Form | Provides the information needed to change the dismissal of your child. All dismissal changes must be in writing, signed, and provided to the school no later than 2:45pm. |
| Transfer of Pupil Record Form | This form is used by incoming students who are transferring to Bakie from another school. |
